- Truthful
- Declare only true facts; a false declaration is an offence.
- First person
- Write in the first person, in numbered paragraphs, and state the facts plainly.
- Purpose
- State the purpose so the receiving institution understands why it is made.
- Before a commissioner
- Sign before a Commissioner for Oaths or notary, who completes the jurat.
Execution requirements
- The declarant signs before a Commissioner for Oaths, who completes the jurat (Statutory Declarations Act; Commissioners for Oaths (Advocates) Act).
- Provide the declaration to the institution and keep a copy.
- Use an affidavit instead if the document is for court proceedings.
